29 CFR 1910.242(a): The employer did not ensure that employees were furnished with tools and equipment that were in a safe condition: A) Mechanic shop at 27 Ridge Rd Whitehouse Station, NJ 08889 -The employer did not ensure the use of a hydraulic cylinder jack in the manner that the jack was designed. The portable jack was used in horizontal directions instead of vertical directions and was not secured in a position to prevent a possibility of slippage of the cap when a portable jack was used to fix a damaged excavator bucket. This condition exposed employees to struck-by hazards. Mechanic shop at 27 Ridge Rd Whitehouse Station, NJ 08889 - Incident occurred on January 3, 2025.

29 CFR 1910.244(a)(2)(i): Where there was a possibility of slippage of the cap, a block was not placed in between the cap and the load in the absence of a firm foundation: A) Mechanic shop at 27 Ridge Rd Whitehouse Station, NJ 08889 - The employer failed to secure the position of the portable jack to prevent the cap from slipping while it was being used to repair a damaged excavator bucket. This failure created a hazardous condition, exposing employees to struck-by risks. Incident occurred on January 3, 2025.
