29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) At 1575 67th Ave. E Sarasota, Florida, 34243; On or about 01-02-2025, the employer did not establish an energy control program consisting of an energy control procedure, training for affected and other employees, and periodic inspections for various machines within the facility to include folders, ironers, washers and dryers therefore exposing the authorized/affected/other employees to nip, pinch, cut, burn, amputate, crush and struck hazards.

29 CFR 1910.303(g)(1):Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: a) At 1575 67th Ave. E Sarasota, Florida, 34243; On or about 01-02-2025, in the big warehouse (2nd Floor) in the sheet folding machine area, northwest corner, employees were exposed to fire and electrical hazards, in that, an electrical panel was blocked by a table and a pallet of boxes, preventing access to it in case of emergency. The minimum clearance of 36 inches was not maintained. b) At 1575 67th Ave. E Sarasota, Florida, 34243; On or about 01-02-2025, in the big warehouse (2nd Floor) in the sheet folding machine area, along west wall, employees were exposed to fire and electrical hazards, in that, an electrical panel was blocked by a cart of tools and material, a bucket, and other loose items preventing access to it in case of emergency. The minimum clearance of 36 inches was not maintained. c) At 1575 67th Ave. E Sarasota, Florida, 34243; On or about 01-02-2025, in the small warehouse (1 st Floor), employees were exposed to fire and electrical hazards, in that, 3 electrical panels were blocked by bins and folded linen preventing access to them in case of emergency. The minimum clearance of 36 inches was not maintained.
