29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Contact heater in the Press Hall at Press 24: On January 3, 2025, the employer exposed employees to amputation, caught-in, and burn hazards, in that the employer failed to ensure the methods of machine guarding were provided for Contact Heater machine when employees placed and retrieved foam sheets within the danger zone of contact heaters with moving parts that were not guarded. b) Contact heater in the Press Hall at Press 21: On or about January 7, 2025, and at times prior, the employer exposed employees to amputation, caught-in, and burn hazards, in that the employer failed to ensure the methods of machine guarding were provided for Contact Heater machine when employees placed and retrieved foam sheets within the danger zone of contact heaters with moving parts that were not guarded. c) Contact heater in the Press Hall at Press 8: On or about January 7, 2025, and at times prior, the employer exposed employees to amputation, caught-in, and burn hazards, in that the employer failed to ensure the methods of machine guarding were provided for Contact Heater machine when employees placed and retrieved foam sheets within the danger zone of contact heaters with moving parts that were not guarded. d) Contact heater in the Press Hall at Press 2: On or about January 7, 2025, and at times prior, the employer exposed employees to amputation, caught-in, and burn hazards, in that the employer failed to ensure the methods of machine guarding were provided for Contact Heater machine when employees placed and retrieved foam sheets within the danger zone of contact heaters with moving parts that were not guarded. e) Contact heater in the Press Hall at Press 1: On or about January 7, 2025, and at times prior, the employer exposed employees to amputation, caught-in, and burn hazards, in that the employer failed to ensure the methods of machine guarding were provided for Contact Heater machine when employees placed and retrieved foam sheets within the danger zone of contact heaters with moving parts that were not guarded. f) Contact heater in the Press Hall at Tesla Front: On or about January 7, 2025, and at times prior, the employer exposed employees to amputation, caught-in, and burn hazards, in that the employer failed to ensure the methods of machine guarding were provided for Contact Heater machine when employees placed and retrieved foam sheets within the danger zone of contact heaters with moving parts that were not guarded.
