All debris was not kept clear from work areas and passageways in and around buildings and other structures.  Site: 1694 Millstone Road, Sag Harbor, NY 11963.  On or before 01/07/2025, employees were exposed to trip and fall hazards while accessing the interior of the home with wooden planks and electrical cords on the floor.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

On or before 01/07/2025, employees were exposed to electrical hazards while using a red 50 ft extension cord used to energize the Bosch Hammer drill that had damaged insulation at the middle section of the cord. 29 CFR  1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers.  Site: 1694 Millstone Road, Sag Harbor, NY 11963.  On or before 01/06/2025, Employees walking and working on the exterior of the site were exposed to falling approximately 11ft into an unprotected opening into the basement of the home.  The exterior hole was partially covered by a board of sheetrock. Site: 1694 Millstone Road, Sag Harbor, NY 11963.  On or before 01/07/2025, employees performing carpentry work in the living room of a two-story residential home. The employees were not utilizing a means of fall protection and was exposed to falling approximately 8 ft to the ground below.  On or before 01/07/2025, employees performing carpentry work outside in the backyard near the outside stairway. The employees were not utilizing a means of fall protection and was exposed to falling approximately 11 ft to the ground below.    Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
