29 CFR  1926.502(d)(17):The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  a) On or about January 8, 2025 at the above address the employees were exposed to fall hazards of approximately of 11 feet ground to eave while performing residential roofing work while not having the attachment point of the safety harness attached at the center of the employees back.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
