On or about January 7, 2025, at a residence located at N2305 Long Cove Drive Waupaca, Wisconsin; the employer did not ensure that an employee in an aerial lift was wearing a personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I.  The employee was working from an elevated basket to remove tree limbs at heights of approximately 50 feet.
