(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:  On or about January 8, 2025, at an establishment located at W3955 Mill Road in Bryant, Wisconsin, employees were exposed to fall hazards in the following instances;   a) While working at the top of the catwalk while replacing the first band saw blade on the two head and were not protected from falls of over 7 feet.  b)  While working at the top of the barn cleaner at the debarker to place the chain back on the sprocket are not protected from falls of over 10 feet.

29 CFR  1910.29(b)(1):(1) The top edge height of top rails, or equivalent guardrail system members, are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface.  The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D-11).  On or about January 8, 2025, at an establishment located at W3955 Mill Road in Bryant, Wisconsin, employees were exposed to fall hazards of over 7 feet while working at the top of the catwalk while replacing the first band saw blade on the two head and were not protected from falls with a guardrail that was too short.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about January 8, 2025, at an establishment located at W3955 Mill Road in Bryant, Wisconsin, the employer did not provide machine guarding to protect operators and other employees from the edger machine while operating the lift platform to feed the edger machine where a crush point was at the operator station.

29 CFR  1910.265(c)(18)(i): Standards. Construction, operation, and maintenance of conveyors shall be in accordance with American National Standard B20.1-1957, which is incorporated by reference as specified in Sec. 1910.6.   At an establishment located at W3955 Mill Road in Bryant, Wisconsin, the employer did not provide machine guarding to protect operators and other employees in the machine areas from hazards during the following instances:  a)  On or about January 8, 2025, in the C Mill on the green chain, the conveyor creates nip points where the guarding has gaps where the chain moves over rollers on the bottom side of the green chain conveyor.  b)  On or about January 8, 2025, on the edger, the feed conveyor is not guarded and has an ingoing nip point near the edger position.  c)  On or about January 8, 2025, on the slab trim, the conveyor is not guarded and has an ingoing nip point on each side of the conveyor belt.  d)  On or about January 8, 2025, in the C Mill at the Roll Case an exposed chain and sprocket were exposing employees to amputation hazards.  e)  On or about January 8, 2025, at the board line chain, three-line chain conveyors have exposed chain and sprockets and were exposing employees to amputation hazards.

At an establishment located at W3955 Mill Road in Bryant, Wisconsin, employees were exposed to caught-in hazards created by the rotating projecting shafts in the following instances;  a)  On or about January 8, 2025, on the green chain, both ends of the green chain conveyor has projecting shaft ends that are projecting more than one half the diameter and were not smooth;  b)  On or about January 8, 2025, on the edger conveyor there is a shaft end projecting more than one half the diameter and was not smooth.
