At the jobsite located at 571 W Kings Highway, Center Hill, FL on or about 12/10/2024, the employer exposed employees to amputation hazards in that guarding provided to the evisceration conveyor was not adequate to prevent the operator from placing hands in the danger zone during the operation cycle and/or when cleaning the machine.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a. At the jobsite located at 571 W Kings Highway, Center Hill, FL on or about 12/10/2024, the employer exposed employees to amputation hazards in that the employer failed to establish an Energy Control Program/Lock Out Tag Out procedure on the Evisceration Conveyor placing employees at risk of amputation hazards.
