29 CFR 1910.184(e)(2)(i):Hooks, rings, oblong links, pear shaped links, welded or mechanical coupling links or other attachments did not have a rated capacity at least equal to that of the alloy steel chain with which they were used or the sling was used in excess of the rated capacity of the weakest component. On or about January 10th, 2025, employees engaged in material handling operations located 3301 G Street, Omaha, Nebraska 68107 were exposed to struck-by hazards in that employees were using Haul Master hoist that was attached to two hooks, one oblong link, and yellow barrel attachment. The yellow attachment, the oblong link, and the two hooks did not contain a rated capacity. Employees were using these components to lift 500lb drums over 9-12 inch high area containment curb.
