29 CFR  1926.451(g)(3)(i): Vertical lifelines were not fastened to a fixed safe point of anchorage, independent of the scaffold, and protected from sharp edges and abrasion.  a) MJ Siding & More LLC at worksite located at 2600 Carronade Drive, Perrysburg, Ohio, 43551: On or about January 16, 2025, the employer did not ensure that employees utilizing personal fall arrest systems while working from a pump jack scaffolding system had vertical lifelines fastened and fixed to a safe point of anchorage.  An employee utilizing a PFAS with a vertical lifeline that was positioned over the aluminum support brace of the pump jack was exposed to falls of 18 ft to the ground below as the lifeline was not maintained independent of the scaffold.
