Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes.   a) On or about November 22, 2024, the employer did not select and require the use of gloves rated for 277 volts, such as Class 00 rubber insulting gloves for an authorized employee exposed to thermal burns from electrical shock while changing out light fixtures on live 277 volts.

On or about November 22, 2024, the employer did not ensure the circuit breaker for the office area lights located in the Penthouse on the roof was properly labeled to indicate its purpose for an authorized employee exposed to 277 volts while changing out light fixtures in the office area, resulting in the authorized employee receiving an electric shock from exposed wires.

Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  a.) On or about January 8, 2025, the employer did not ensure the manufacturer's prior written approval was received and ensured that the capacity, operation, and maintenance instruction plates, tags, or decals were changed accordingly for CAT sit-down forklifts with Arrow trailer spotter attachments which were used by forklift operators to move trailers.
