29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects. a) Worksite: On or about 01-07-2025, ground worker employees using tag lines were exposed to struck-by hazards from not utilizing head protection while manually lowering an aluminum staging arch measuring approximately 27-feet in width and 20-feet in height.
