29 CFR 1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work.  a) Jobsite: On or about 1/13/2025, employees were exposed to electrical type hazards when working from an extension ladder that was set-up underneath the live detached electrical power feed coming into the structure, where the employees were expected to work within inches of the live electrical wire.

29 CFR  1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  a) Jobsite: On or about January 13, 2025, the employer allowed two employees to use damaged ladders, with bent and dented rungs.
