Shoop Ave. Wauseon, OH: Employees performing service and maintenance task(s) on exhaust fan #14 in the "E.S. Supply Room" were exposed to caught in hazards in that the employer did not develop, document, or utilize procedures to verify and control the hazardous energy created by the moving components of the motor's drive belt and pulley.
