Douglas Street, Philadelphia, PA - On or about January 13, 2025, the employer did not ensure employees engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net system, or personal fall arrest system. Employees were performing roofing approximately 20 feet above ground level without the use of a fall protection system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Jobsite at or near 1723-1731 S. Douglas Street, Philadelphia, PA - On or about January 13, 2025, the employer did not provide fall protection training to employees who were exposed to fall hazards. Employees were performing roofing activities from heights greater than 6 feet without being trained on fall protection, thereby exposing employees to fall hazards.
