29 CFR 1926.451(b)(4): Each end of a platform, which was not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches. Location - Water treatment building. On or about January 13, 2025, two employees were exposed to fall hazard while working/walking on a pump jack scaffold where the pick/platform did not extend the minimum of six inches over center line of brace/bracket. The pick/platform fell causing the employee to fall and was injured.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee. (a) Location - Water treatment building. east side of building. On or about January 13, 2025, where ladder jack scaffold was located, two lifelines were attached to one roof anchor. One employee was already attached to one of the lifelines. A second employee was going to attach to the second lifeline but fell prior to attaching to second lifeline. Employee(s) were not protected from falling when using personal fall protection system which did not have an anchor point of supporting at least 5000 pounds. (b) Location - Water treatment building. east side of building On or about January 13, 2025, where ladder jack scaffold was located, two lifelines were attached to one roof anchor. The roof anchor was not secured per manufacturers instruction. The roof anchor only had two fasteners on the visible leg. Manufacturer requires minimum of six fasteners per leg. Employee(s) were not protected from falling when using personal fall protection system which did not have an anchor point of supporting at least 5000 pounds. (c) Location - Water treatment building. west side of building On or about January 13, 2025, where pumpjack scaffold was located, three lifelines were attached to one roof anchor. Employee(s) were not protected from falling when using personal fall protection system which did not have an anchor point of supporting at least 5000 pounds.
