1910.147(d)(5)(i) All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: The employer failed to develop an adequately written energy control procedures for employees to follow while servicing and maintaining the Extrusion Mill HHV-1 Modig Machine #W0290, Serial #130421 at the Warehouse 3-193G by Column D14. Employees were exposed to amputation in that the equipment specific energy control procedures did not include steps to isolate the magnet energy.
