29 CFR  1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).  On or about January 16, 2025, employees were exposed to struck-by hazards as the Powered Industrial Truck (PIT) operator was not certified or licensed to operate a PIT.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  On or about January 16, 2025, employees were not provided with a safe means of access to a ladder jack scaffold, exposing the employee to a 12 foot fall.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.451(f)(7): Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person.  On or about January 16, 2025, scaffolds were not erected under the supervision of a competent person qualified in scaffold erection, exposing the employee to the hazards associated with scaffold work, such as fall, and collapse.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.502(d)(6)(iii): Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged: to a dee-ring to which another snaphook or other connector is attached.  On or about January 16, 2025, employees attached two (2) personal fall arrest snaphooks to one (1) dee-ring,  exposing the employee to the hazards associated with an unintentional disengagement.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   On or about January 16, 2025, employees were not not protected from a fall while performing framing operations from a scaffold, exposing the employees to a fall of 12 feet.  The Tricor Carpentry LLC dba Tricor  commercial Carpentry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 1655878, citation number 1, item number 1 and was affirmed as a final order on July 25, 2023, with respect to a workplace located at16200 S. Harlem Ave. Tinley Park, IL 60477.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
