OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that proper equipment was not used to lift prefabricated trusses. (a) Coccomo Brothers Associates, LLC, 57 Club Drive, Berlin, CT 06037; On or about January 16, 2025, and at times prior the employer did not follow manufacturer's recommendations during the installation of prefabricated metal plate joined wood trusses regarding handling of the truss. The employer did not ensure a spreader bar and the proper rigging equipment was used to lift the truss.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) Coccomo Brothers Associates, LLC, 57 Club Drive, Berlin, CT 06037; On or about January 16, 2025, and at times prior, employees were working under a suspended load that was lifted by an excavator bucket while working on a roof framing project. Employees were not wearing head protection and were exposed to severe head injuries secondary to struck-by or crushing hazards.
