29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about January 16, 2025, in the eastern section of the trench, a two-piece extension ladder was separated, and the separate sections were used individually to access a trench by employees performing utility work.
