29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that: a) On or about January 8, 2025, at the Tampa Campus of Southern Technical Institute, located at 3910 Riga Boulevard, Tampa, FL, in or near the main electrical service room: The line side of the main service panel which is a 3-phase electrical system was not tested and determined to be deenergized prior to an employee connecting a 400amp temporary service line to provide electricity to the welding laboratory exposing the employee to electrical shock and burn hazards.
