29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  At the machine shop, on the north end, employees engaged in machine activities were exposed to health hazards in that the employer did not label a secondary container of Hysol MB 20 (Metalworking Fluid), a hazardous chemical used for metalworking, that can cause serious eye irritation, skin irritation, and is carcinogenic to humans.

29 CFR  1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  In the Assembly Area, employee(s) engaged in material handling activities while operating Nissan forklift (s) were exposed to struck-by and crush-by hazards in that the wheels/tires on the Nissan 60 were found to be in disrepair with several cracks, fissures, and pieces missing. The Nissan 14 was also found to be in disrepair in that the back -up sound/lights were inopperable.
