29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a)	On or about January 11, 2025, at the facility located at 490 Quality Court, Wrightstown, Wisconsin; the employer did not ensure that all employees engaged in servicing and maintenance tasks associated with the SML Extruder (Machine No.: 2024.4003.01.01/CC/514) were trained in the methods and means necessary for energy isolation and control when entering hazardous areas.  Specifically, an employee trained as affected was engaged in threading the web, a servicing and maintenance activity and was not trained as an authorized employee.  b)	On or about January 16, 2025, the employer did not ensure that all employees engaged in servicing and maintenance tasks associated with the SML Extruder (Machine No.: 2024.4003.01.01/CC/514) were trained in the methods and means necessary for energy isolation and control when entering hazardous areas.  Specifically, an employee trained as affected was engaged in removing plastic stuck to the take-off roller, a servicing and maintenance activity and was not trained as an authorized employee.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) On or about January 11, 2025, employees were exposed to machine hazards associated with moving parts when they engaged in re-threading activities on the SML Extruder (Machine No.: 2024.4003.01.01/CC/514).  The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to rethread the plastic film.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity exposing employees to potential machine hazards.  b) On or about January 16, 2025, employees were exposed to machine hazards associated with moving parts when they engaged in removal of plastic film wrapped around rolls on the SML Extruder (Machine No.: 2024.4003.01.01/CC/514).  The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to remove the plastic film.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity exposing employees to potential machine hazards.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.
