29 CFR 1926.1051(a):Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: Worksite located at 102 NW Business Parkway, Riverside, Missouri- An employee was exposed to a fall hazard. The employee had to climb off of an attached caged ladder down to a step ladder that was more than 5' below the bottom rung of the attached ladder.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder shall not be used as a step: Worksite located at 102 NW Business Parkway, Riverside, Missouri- Employee was exposed to a fall hazard. The employee was dismounting off a fixed ladder onto a stepladder in which the employee was standing on top of the top two steps that are not to be used as a step.
