29 CFR  1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  (a) On or about January 28, 2025, the employer did not institute a respiratory protection program for employees that were required to wear 3M P95 respirators while working in the mold press and welding areas of the facility.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR  1910.134(e)(1):The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a) On or about January 17, 2025, within the molding and welding departments, the employer did not provide a medical evaluation to determine each employee's ability to use a respirator before the employee was required to use a 3M P95 respirator,  M/N: 8577 in the workplace.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. 29 CFR  1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):  (a) On or about March 25, 2025, the employer did not ensure that employees in the Molding and Welding Areas of the facility, who were required to wear 3M P95 respirators, M/N: 8577, were fit tested to ensure proper sizing and efficacy of the respirators used at the facility. 29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) On or about January 28, 2025, employees within the mold department were not provided effective information and training on hazardous chemicals in their work area or the location of Safety Data Sheets within the establishment. Employees were exposed to chemical contact hazards while working with chemicals including mold releases which contain volatile organic chemicals and organic tin within the products applied to mold presses used by the operators.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification.
