29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: On or about January 15, 2025, at the IRCA Group USA facility in Afton Missouri, at the Lower Panning Area, the employer did not implement an annual LOTO inspection on the mixing machine, resulting to an employee to sustain an amputation of the Right middle finger while troubleshooting the clogged mixing machine when his finger got caught with the rotating part of the machine. The affected employee was not properly taught how to apply the LOTO, nor an annual inspection or review of LOTO procedures when troubleshooting the Mixing Machine.

29 CFR 1910.147(c)(7)(i):The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: On or about January 15, 2025, at the IRCA Group USA facility in Afton Missouri, at the Lower Panning Area, did not provide adequate LOTO training when troubleshooting the mixing machine, resulting to an employee who sustained an amputation of the Right middle finger while troubleshooting the clogged mixing machine. His finger got caught with the rotating part of the machine without applying LOTO procedures specifically switching-off the electrical power to this mixing machine.
