An extended ladder was instead used, which exposed employees to 15 foot fall hazards. Violation occurred on or about January 17, 2025.

Employees used a pump jack scaffold to carry out framing and sheathing work at a height of 15-foot with no means of fall protection. Violation occurred on or about January 17, 2025.
