29 CFR  1910.157(c)(1):The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.  a. On or about January 27, 2025, located at 47201 County Road 273, Conesville, Ohio 43811, portable fire extinguishers located on every other transformer pad were not mounted or identified to ensure they were readily accessible to the employees. As a result, employees were exposed to electrical fire hazards.

On or about January 27, 2025, located at 47201 County Road 273, Conesville, Ohio 43811, the employer used extension cords to supply 240 VAC to the heat trace lines for the water lines. The cords were rated for single phase 120 VAC applications in accordance with the manufacturers instructions. As a result, the employees were exposed to an electrical shock hazard.

On or about January 27, 2025, located at 47201 County Road 273, Conesville, Ohio 43811, the employer used extension cords to supply 240 VAC to the heat trace lines for the water lines. The receptacles were exposed to wet outdoor conditions and did not have ground-fault circuit-interrupter (GFCI) protection. As a result, the employees were exposed to an electrical shock hazard.
