29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On the shop floor located at 4500 110th Ave. N Clearwater, FL. On or about 12/06/2024, the employer exposed employees to amputation and caught in-between hazards, in that the Amada Shear Press was not adequately guarded while employees operated the machine.
