29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about January 22, 2025, the employer did not ensure that machine specific energy control procedures were developed, implemented, and utilized for equipment including but not limited to Giddings & Lewis - Fraser Horizontal Mills (70A-D5T, serial #150-820), (60 DP5T, Serial #150-1488) and a Momura Machine Tool Works Horizontal Mill, which had multiple sources of energy including electrical (480V), and hydraulic.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   a.	On or about January 22, 2025, the employer did not ensure operator(s) of the: Giddings & Lewis - Fraser Horizontal Mills (70A-D5T, serial #150-820) were protected by one or more methods of machine guarding from the moving and rotating parts of the chuck and cutting blades of the mill. The operator removes the Chip Guard/s for small (in diameter) orders and operates the machine. The operator removes the hinged plexiglass panel and sets it off the side while the employee operates the machine. The employees are also exposed to caught in/caught between hazards from unguarded rotating parts and pinch points. The bed ways (2 axels that power the main table, rotary table, and angle plate) were open and unguarded. Employees routinely access the area above and around the bed ways and bed rails to check the milling process throughout their regular shift as a machinist operating the horizontal mill.  b.	On or about January 22, 2025, the employer did not ensure operator(s) of the: Giddings & Lewis - Fraser Horizontal Mills (60 DP5T, Serial #150-1488) were protected by one or more methods of machine guarding from the moving and rotating parts of the chuck and cutting blades of the mill. The operator removes the Chip Guard/s for small (in diameter) orders and operates the machine. The operator removes the magnetic panels and sets it off the side (attached to a lubricant metal barrel) while the employee operates the machine. The employees are also exposed to caught in/caught between hazards from unguarded rotating parts and pinch points. The bed ways (2 axels that power the main table, rotary table, and angle plate) were open and unguarded. Employees routinely access the area above and around the bed ways and bed rails to check the milling process throughout their regular shift as a machinist operating the horizontal mill.  c.
