29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 1/08/2025, at the jobsite located at 5405 Webster Avenue West Palm Beach, FL 33405, employees were exposed to a caught by and amputation hazard while operating a Powermatic 14" bandsaw (Model No. PWBS-14 Stock No. 1791216K) which had the unused portion of the blade not completely guarded. b) On or about 1/08/2025, at the jobsite located at 5405 Webster Avenue West Palm Beach, FL 33405, employees were exposed to a caught by and amputation hazard while operating a Powermatic bandsaw (Serial No. 2-1759) which had the guide rolls and unused portion of the blade not completely guarded. c) On or about 1/08/2025, at the jobsite located at 5405 Webster Avenue West Palm Beach, FL 33405, employees were exposed to struck by and caught by hazards while operating a Bridgeport milling machine (Serial No. Unknown) that lacked a guard.
