29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1): a) On or about January 23, 2025, at an establishment located at 280 North Mayflower Road, Appleton, WI 54913, the employer did not ensure that examinations of electrical equipment were conducted to ensure that equipment was free from recognized hazards when the rigid metal conduit above the Rotary Automotive Lift had been struck and damaged. b) On or about January 23, 2025, at an establishment located at 280 North Mayflower Road, Appleton, WI 54913, the employer did not ensure that examinations of electrical equipment were conducted to ensure that equipment was free from recognized hazards when the polymer conduit leading into a junction box outside the mechanical room had broken and exposed the interior wires.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a) On or about January 23, 2025, at an establishment located at 280 North Mayflower Road, Appleton, WI 54913, the employer did not ensure that a metal junction box in the carwash area was equipped with a cover to prevent water and other material entering the area around live electrical wires. b) On or about January 23, 2025, at an establishment located at 280 North Mayflower Road, Appleton, WI 54913, the employer did not ensure that LEDVANCE LED Canopy Light in the detailing area was equipped with a cover to prevent water and other material entering the area around live electrical wires.
