29 CFR 1910.334(d): Where flammable materials were present only occasionally, such as but not limited to flammable gases, vapors, or liquids; combustible dust; and ignitable fibers or flyings, electrical equipment capable of igniting them shall not be used;     a) On or about 01/23/2025 in the New Production Room 4; employees were exposed to explosion hazards when a Porter Cable brand wet/dry vacuum (model PCX18301-4B), not approved for combustible dust, was used to vacuum the equipment where fugitive dust (combustible dust) was present. 29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 01/21/2025 in the New Production Room 4, where an employee reached into the sprocket wheels and chains of the SANKO machine Line #73 to clean excessive sugar dust while the machine was running, the employee was exposed to the nip point between the sprocket wheels and chains.  b) On or about 01/23/2025 in the New Production Room 4, where employees performed weekly cleaning, unjamming, and setup of the SANKO machines, requiring the removal of guards and disassembly of parts without locking out or controlling energy sources, employees were exposed to caught-by hazards in the event of unexpected machine start-up while reaching inside to remove components.  ABATEMENT DOCUMENTATION REQUIRED  The American Sugar Refining, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1597531, citation number 1, item number 1 and was affirmed as a final order on 09/09/2022, with respect to a workplace located at 692 Bailey Avenue, BUFFALO, NY 14206.

The employees performing the covered operations on the SANKO machines had not been trained as authorized employees in the applicable Energy Control Procedures.  ABATEMENT DOCUMENTATION REQUIRED  The American Sugar Refining, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 1597531, citation number 1, item number 3 and was affirmed as a final order on 09/09/2022, with respect to a workplace located at 692 Bailey Avenue, BUFFALO, NY 14206.
