29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks: a) On or about January 23, 2025, at the production area of the jobsite located at 1061 East 32nd Street, Hialeah, Florida 33013, an employee was exposed to an amputation hazard while a laminator machine (i.e. D&K- Model No. AccuFeed 32 - Serial No. 32-5249) was operated without a guard.
