29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: (a) On January 23, 2025, employees installed a septic tank riser in an excavation without cave-in protection. The excavation measured approximately 5.7' deep and was composed of Type B soil. No inspections of the excavation were made by a competent person prior to workers entering.
