29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a- 307 Taft Drive, Woolwich Township NJ: Workers were exposed to fall hazards, in excess of 14 feet 5 inches, when performing roofing related activities from 10 on 12 pitched roofs while wearing shoulder straps that were field modified, with carabiners, to attach to their body belts. b - 307 Taft Drive, Woolwich Township NJ: Workers were exposed to fall hazards, in excess of 14 feet 5 inches, when performing roofing related activities from 10 on 12 pitched roofs while wearing field modified shoulder straps that were attached to body belts with carabiners that were in turn connected to lanyard's rope grab mechanisms. c - 307 Taft Drive, Woolwich Township NJ: The company owner was exposed to fall hazards, in excess of 14 feet 5 inches, when performing roofing related activities from a 10 on 12 pitched roof while wearing a full body harness that was not supplied with the proper connection means to the worksite lanyards. d - 307 Taft Drive, Woolwich Township NJ: The company owner and workers were exposed to fall hazards, in excess of 12 feet 2 inches, when accessing a portable extension ladder to egress pitched roofs, while not utilizing fall protection.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: a- 307 Taft Drive, Woolwich Township NJ: Workers and the company owner were exposed to fall hazards, in excess of 12 feet 2 inches, when accessing to and egressing from a residential structure's pitched roofs using a damaged aluminum extension ladder. Condition noted on or about 01/23/25.
