29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. At the Main Shop area of the machine shop, located at 2901 Danese Street, in Jacksonville, FL: On or about January 23, 2025, the employer exposed employees to caught in and amputation hazards, in that the employee was exposed to rotating parts while performing milling work on a Lathe American machine tool Turnmaster 17x 60, without a chuck guard installed. b. At the CNC area of the machine shop, located at 2901 Danese Street, in Jacksonville, FL: On or about January 23, 2025, the employer exposed employees to caught in and amputation hazards, in that the employee was exposed to rotating parts while performing milling work on a Lathe Summit machine tool S/N 8551, without a chuck guard installed.

29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses: a. At the CNC area of the machine shop, located at 2901 Danese Street, in Jacksonville, FL: On or about January 23, 2025, the employer failed to ensure that employees who wore prescription glasses were protected from eye injuries, in that eye protection that incorporated the prescription in its design or eye protection that can be worn over the prescription lenses were not enforced while employees performing various tasks with different metals such as but not limited to drilling, milling and cutting.
