OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that were exposed to struck-by and crushing hazards. a) On or about January 23, 2025, employees were exposed to struck-by and caught-in hazards of falling loads while installing radiators on the ICE line. Employees were working underneath suspended loads to remove bolts prior to placing the radiator onto the frame.

29 CFR 1910.184(d): Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service. a) Located at 1951 Reiser Avenue Southeast, New Philadelphia, OH 44663: On or about January 23, 2025, the employer did not ensure daily inspections of a Liftex 12-foot nylon sling were performed on the ICE line prior to using the sling to lift frames onto the assembly line
