This exposed employees to fall hazards due to: a. Fork mounted platform not designed by a registered engineer b. No constructed with complete material, holes cut into the plywood c. Anchor points made from two by four nailed across the back corner of the fork mounted box

29 CFR 1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about January 23, 2025, and at times prior to, the employer did not provide their employees with fall protection when working at elevated heights. This conditions exposed employees to fall from the A-frame ladder use on the second story balcony that did not have a complete guard rail.
