29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Carbon Monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 ppm: a) At the jobsite located at 942 Heritage Pass, Davenport, Florida: On or about January 23, 2025, an employee inside the trailer was exposed to 4196.2 ppm of carbon monoxide (CO) over a 60-minute period, based on carboxyhemoglobin level in the employee's blood. This is equivalent to an 8-hour TWA exposure of 2524.5 ppm, which represents 10.49 times the OSHA-PEL of 50 ppm.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) At the jobsite located at 942 Heritage Pass Davenport, Florida: On or about January 23, 2025, effective information and training was not provided to the employees when they worked with hazardous chemicals, including Gasoline, Diesel and Propane and were exposed to combustion by-products, including Carbon Monoxide, generated from the burning of fuel.
