29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Axis Redistribution, LLC- WP Innova - Model Number: MH-FG-2000 A, Serial Number: WR2101214 located at 3775 Fancy Farms Rd, Plant City, Florida: On or about January 24, 2025, the employer did not ensure that one or more methods of machine guarding such as an interlocked barrier guard, fixed barrier guard and or a presence sensing device were provided to protect employees from struck-by hazards created by a stretch film wrapping machine at the point of operation and rotating parts. b) Axis Redistribution, LLC- WP Innova - Model Number: MH-FG-2000 A, Serial Number: WR2208153 located at 3775 Fancy Farms Rd, Plant City, Florida: On or about January 24, 2025, the employer did not ensure that one or more methods of machine guarding such as an interlocked barrier guard, fixed barrier guard and or a presence sensing device were provided to protect employees from struck-by hazards created by a stretch film wrapping machine at the point of operation and rotating parts. c) Axis Redistribution, LLC- WP Innova - Model Number: MH-FG-2000 A, Serial Number: WR2003236 located at 3775 Fancy Farms Rd, Plant City, Florida: On or about January 24, 2025, the employer did not ensure that one or more methods of machine guarding such as an interlocked barrier guard, fixed barrier guard and or a presence sensing device were provided to protect employees from struck-by hazards created by a stretch film wrapping machine at the point of operation and rotating parts. d) Axis Redistribution, LLC- WP Innova - Model Number: MH-FG-2000 A, Serial Number: WR2107122 located at 3775 Fancy Farms Rd, Plant City, Florida: On or about January 24, 2025, the employer did not ensure that one or more methods of machine guarding such as an interlocked barrier guard, fixed barrier guard and or a presence sensing device were provided to protect employees from struck-by hazards created by a stretch film wrapping machine at the point of operation and rotating parts.
