29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  At the jobsite located at 2548 S 42nd Street Kansas City, Kansas, employees engaged in commercial roofing activities, walking and working on a  roof, were exposed to fall hazards of approximately 15 feet, 7 inches from the eave of the roof to the ground below in that fall protection was not utilized.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsite located at 2548 S 42nd Street Kansas City, Kansas, employees performing roofing work were exposed to fall hazards of approximately 15 feet 7 inches in that employees had not received fall protection training on how to minimize these hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  At the jobsite located at 2548 S 42nd Street Kansas City, Kansas, employees engaged in commercial roofing activities, using a Keller 24 foot metal extension ladder, were exposed to fall hazards of approximately 15 feet from the eave of the building to the ground below. Employees were exposed in that the ladder did not extend at least 3 feet above a landing surface.
