OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing and struck-by hazards from falling vehicles and/or automotive lift parts: a. Site: On or about April 24, 2025, and at times prior; employees operating a Nussbaum JUMBO-LIFT II NT, Serial No 238399, hydraulic automotive lift were not protected from crushing and struck-by hazards where the lift was not inspected by a qualified lift inspector within the previous 12 months, repair maintenance was not accomplished in response to deficiencies reported by the most recent annual inspection report, the manufacturer's operator/maintenance manual and the ALI safety manual and safety tips were not readily available to the operators. b. Site: On or about April 24, 2025, and at times prior; employees operating Nussbaum JUMBO 7, Serial No 15022401, 15022403, 15022404, 15022405, and Bay 6; hydraulic automotive lifts were not protected from crushing and struck-by hazards where the lifts were not inspected by a qualified lift inspector within the previous 12 months,, repair maintenance was not accomplished in response to deficiencies reported by the most recent annual inspection report, the manufacturer's operator/maintenance manual and the ALI safety manual and safety tips were not readily available to the operators. c. Site: On or about April 24, 2025, and at times prior; employees operating Ben Pearson 9000SA; Serial No 01029518, 080210508, 080210516, 070210392, 070210419, 070210343, 01029517, and Bay 12; 2 post automotive lifts were not protected from crushing and struck-by hazards where the lifts were not inspected by a qualified lift inspector within the previous 12 months,, repair maintenance was not accomplished in response to deficiencies reported by the most recent annual inspection report, the manufacturer's operator/maintenance manual and the ALI safety manual and safety tips were not readily available to the operators. d. Site: On or about April 24, 2025, and at times prior; employees operating Nussbaum SPL 9000; Serial No 232468 and 238399; 2 post automotive lifts were not protected from crushing and struck-by hazards where the lifts were not inspected by a qualified lift inspector within the previous 12 months, repair maintenance was not accomplished in response to deficiencies reported by the most recent annual inspection report, the manufacturer's operator/maintenance manual and the ALI safety manual and safety tips were not readily available to the operators. e. Site: On or about April 24, 2025, and at times prior; employees operating Rotary SPOA7; Serial No ALW99C0003 and ABB01F0074; 2 post automotive lifts were not protected from crushing and struck-by hazards where the lifts were not inspected by a qualified lift inspector within the previous 12 months, repair maintenance was not accomplished in response to deficiencies reported by the most recent annual inspection report, the manufacturer's operator/maintenance manual and the ALI safety manual and safety tips were not readily available to the operators. f.
