29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a). In the manufacturing plant, ACCUR Shear machine Model No.625012, Serial No.7789: On or about January 28, 2025, the employer did not ensure that one or more methods of machine guarding such as an interlocked barrier guard, fixed barrier guard and or a presence sensing device were provided to protect employees from struck-by hazards created by releasing cut material in the rear of the machine and rotating parts. b). In the manufacturing plant, Piranha Ironworks Machine, Serial No.9381: On or about January 28, 2025, the employer did not ensure that one or more methods of machine guarding such as an interlocked barrier guard, fixed barrier guard and or a presence sensing device were provided to protect employees from caught-in hazards created when releasing cut material on the discharge area of the machine and the foot pedal does not protect at point-of-operation. c). In the manufacturing plant, COE machine MODEL.625127, Serial No.103-04-20: On or about January 28, 2025, the employer did not ensure that one or more methods of machine guarding such as an interlocked barrier guard, fixed barrier guard and or a presence sensing device were provided to protect employees from caught-in hazards created because the fixed barrier guard allows operators to place hands in danger zone while machine is operating. d). In the manufacturing plant , Atech FRP saw-Mod.crater A550 Serial No. 840224: On or about January 28, 2025, the employer did not ensure that one or more methods of machine guarding such as an interlocked barrier guard, fixed barrier guard and or a presence sensing device were provided to protect employees from caught-in hazards created when the saw is in the closed position, the openings on the side are not guarded when an additional operator is assisting from the side of the machine. e). In the manufacturing plant, Cincinnati Model No.175CBX12FT, Serial No.45916: On or about January 28, 2025, the employer did not ensure that one or more methods of machine guarding such as an interlocked barrier guard, fixed barrier guard and or a presence sensing device were provided to protect employees from caught-in hazards in the rear of the machine and the foot pedal does not protect at point-of-operation. f). In the manufacturing plant, Roper Model No.AB1212KF2, Serial No.7789: On or about January 28, 2025, the employer did not ensure that one or more methods of machine guarding such as an interlocked barrier guard, fixed barrier guard and or a presence sensing device were provided to protect employees from caught-in hazards, in that the foot pedal does not protect at point-of-operation. g). In the manufacturing plant area, W.F. Wells band saw, Model No. AC-7, Serial No. AC1780 : On or about January 28, 2025, the employer exposed employees to amputation and caught in-between hazards, in that the saw was not adequately guarded while employees operated the machine.
