Packing Area: On or about January 28, 2025, and at times prior, the employer exposed employees to electrical shock hazards, in that an unused opening was not closed on the top of a 4 inch by 4 inch electrical box containing two electrical receptacles which was mounted to a table and used by employees to charge cellular phones and supply power to a space heater while performing packing duties.

Packing Area: On or about January 28, 2025, and at times prior, the employer exposed employees to electrical hazards, in that the employer used a flexible cord as a substitute for fixed wiring to provide electrical power from a ceiling mounted receptacle to a table mounted receptacle which was used by employees to charge cellular phones and to supply power to a space heater while performing packing duties. b. Saw Area: On or about January 28, 2025, and at times prior, the employer exposed employees to electrical hazards, in that the employer used a flexible cord as a substitute for fixed wiring to provide electrical power from a ceiling mounted receptacle to a column mounted receptacle to energize a "DeWalt" model DW713 compound miter saw (serial number 397600).
