29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On January 28, 2025, at the above addressed jobsite, employees performing demolition work on residential home, approximately 10 feet and 7 inches above the ground below, were not secured to personal fall arrest systems or other means of fall protection, thereby exposing the employees to fall hazards. b) On January 28, 2025, at the above addressed jobsite, employees performing demolition work on residential home, approximately 10 feet and 8 inches above interior second floor, were not secured to personal fall arrest systems or other means of fall protection, thereby exposing the employees to fall hazards. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On January 28, 2025, at the above addressed jobsite, the Werner A-Frame Portable Ladder used by employees to access and egress the roof of the house, measuring approximately 10 feet 8 inches from the floor of the second story to the working platform on the turret, was not extended to have the side rails extend at least three (3) feet above the upper landing. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
