29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) Location: 19 Vine St, Weymouth, MA On or about January 28, 2025, frequent and regular inspections were not adequately conducted to identify and correct worker exposure to fall hazards where fall protection was not utilized and to identify and correct worker exposure to fall hazards and crushed-by hazards when workers were conducting framing activities. The house being framed lacked sufficient lateral bracing and shear resistance during construction and collapsed during construction resulting in a fatality and injuries to several workers.

The worker had been exposed to fall hazards of up to 25 feet to the ground prior to the collapse and did not have fall protection training to recognize and minimize fall hazards.
