In the warehouse area, employees engaged in forklift operation activities were exposed to struck-by hazards in that forklift training had not been completed to include formal instruction, practical training and certification.

Employees were handling chemicals including but not limited to Final Rinse, Platinum Plus, and Vital Oxide. The hazards associated with these chemicals include skin and eye irritation.
