29 CFR  1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  (a) In the extrusion department, an operator working at machines 3 and 5 was exposed to noise levels at 99.5% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 99.5% is approximately 89.9 dBA.  The average sound level was 87.1 dBA, which exceeds the calculated reduced action level of approximately 82.2 dBA for the shift greater than 8 hours.  Sampling was performed for 709 minutes during one shift on February 19, 2025.    (b) In the extrusion department, an operator working at machines 4 and 6 was exposed to noise levels at 128.4% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 128.4% is approximately 91.8 dBA.  The average sound level was  88.9 dBA, which exceeds the calculated reduced action level of 82.2 dBA for the shift greater than 8 hours.  Sampling was performed for 709 minutes during one shift on February 19, 2025.    (c) In the extrusion department, an operator working at machines 7 and 8 was exposed to noise levels at 131.6% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 131.6% is approximately 91.9 dBA.  The average sound level was 89.1 dBA, which exceeds the calculated reduced action level of approximately 82.2 dBA for the shift greater than 8 hours.  Sampling was performed for 710 minutes during one shift on February 19, 2025.    (d) In the extrusion department, a supervisor was exposed to noise levels at 76.4% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 76.4% is approximately 88 dBA.  The average sound level was 85.2 dBA, which exceeds the calculated reduced action level of approximately 82.2 dBA for the shift greater than 8 hours.  Sampling was performed for 706 minutes during one shift on February 19, 2025.    (e) In the printing department, an operator working at press 2 was exposed to noise levels at 101.2% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 101.2% is approximately 90 dBA.  The average sound level was 87.3 dBA, which exceeds the calculated reduced action level of approximately 82.2 dBA for the shift greater than 8 hours.  Sampling was performed for 703 minutes during one shift on February 19, 2025.    (f) In the printing department, a helper working at press 2 was exposed to noise levels at 96.8% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 96.8% is approximately 89.7 dBA.  The average sound level was 86.9 dBA, which exceeds the calculated reduced action level of approximately 82.2 dBA for the shift greater than 8 hours.  Sampling was performed for 708 minutes during one shift on February 19, 2025.    (g) In the printing department, an operator working at press 4 was exposed to noise levels at 113.9% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 113.9% is approximately 90.9 dBA.  The average sound level was 88.1 dBA, which exceeds the calculated reduced action level of approximately 82.2 dBA for the shift greater than 8 hours.  Sampling was performed for 703 minutes during one shift on February 19, 2025.
