At the jobsite located at 6110 Woodstock Street, Shawnee, Kansas, employees using a portable 24 foot extension ladder to access the roof to perform roofing activities were exposed to fall hazards of approximately 11 feet from the roof to the ground. The ladder was not secured and the side rails of the portable extension ladder did not extend at least 3 feet above the landing surface of the roof.  The side rails extended approximately 2 feet.
